This unique organization brings together family doctors and other healthcare providers to create individual wellness programs for our residents based on their personal needs. The Rapids Family Health Team will provide a primary care doctor for those seniors who don’t have one, plus develop programs to promote good health and self-care to help residents better manage their own health.
